Due to a mistake that led to the death of a patient, Seton Medical Center in Daly City has been fined $100,000, which is the maximum penalty and fine under state law.

According to a California Department of Public Health (CDPH) investigation, a citation was issued when an 81-year-old woman suffocated and died after a nurse failed to remove the cap on the breathing tube that was placed in the trachea of the woman.

Seton officials plan to appeal the penalty. They stress that providing a safe and caring environment is fundamental to their mission at Seton Medical Center and that they are saddened by all patient losses.
